I used one of these for approximately 9 months. It did improve my airway/sleep, and it did work in conjunction with CPAP but required a full face mask.

The downside is that it ruined my bite creating touching incisors. I admit though, I was using a lot of advancement to make it work. Your mileage may vary.

The main thing it did for me is prove that opening my airway at base-of-tongue was the source of my problem. I used that info to decide on MMA which solved my problem.
