Since the question about two phase protocol was
raised, let me respond by saying that I do not
believe in it and don't advocate it. All of these
sleep apnea surgical procedures have been done
over and over again for many years. We know the
probability of success the procedures and we know
the variables that affect outcome. The idea of
"trying" to use the least invasive procedure (we
all know how invasive UPPP is) to improve sleep
apnea may be very psychologically attractive to
patients, but is not sound. Examining the patient
and assessing all the factors, discussing risks
with the patient and formulate the probability of
success with different surgical options is what
surgeons should do. Following a protocol for
every patient is not sound medical practice,
especially in sleep apnea surgery.
