I have been using a mouthpiece for 10 years and it is still effective, however, I do not have any jaw pain and had mild to moderate apnea. You are still young and MMA surgery is a very serious decision. Read everything on this site about results people have had (not always good) before you stop something that is working for you now. I would at least give the mouthpiece a few years or until it stops working. Meanwhile do your research..sleep well my friend.
