Perhaps you can persuade your gastro to write the prescription for the study in spite of the pulmonologist. I don't think that the law requires a pulmonologist to write the sleep study request.

After a failed sleep study (It was an invalid study.) I paid for a study at another sleep center out of my pocket and then got reimbursed.

At that time there were about 6 study centers within an hour's drive of home. (I had to take public transportation because my license was suspended.)
