There are a lot of people that do just fine with being compliant. Then there are others where it is too much trouble, they don't like it or whatever the reason. Most of these fall into the mild to moderate range of OSA. The more severe OSA patients usually take it quite serious and are more compliant. They generally already know that OSA is progressive and life threatening.

Unfortunately as these non compliant people get older OSA continues to get worse. They will blame their hypertension,GERDS or heart disease etc. on everything else but OSA.

Lying to doctors is a childs game. Tell them to grow up. You would think that they would prefer CPAP or doing whatever they have to do to treat their OSA. The alternative is pushing up daisies.
Seems like a no brainer to me.
