Price range from $500 ~ $2500 and it's fitted thru a dentist who does this sort of thing: http://www.dentalsleepmed.org/

I use the TAP. It cost around $650. It helps a little, but only after I had surgeries that helped open the airway at the level of the tongue (genioplasty/hyoid advancement). Before this the dental apliances didn't do a thing. It's effective for mild to moderate apnea. Only opens up the airway slightly. I've also tried the Modified Herbst ($1350) and this was unconfortable and didn't help. I hear the most comfortable is the Silencer ($2500), but it's very expensive.
