It's definitely common for insurance companies to require CPAP before they'll pay for surgery. However, if you have a certain condition that prevents you from using CPAP, the doctor should be able to write a letter to the insurance company so that they will approve.

Either way, your doctors office should have known before they scheduled you for surgery. In my case, they told me right up front that CPAP had to be tried first before the insurance company would even consider it. I'd make the office or doctor do some work now to try and remedy the situation.

Good luck.
