Actually, back in...August 1997, when I had my first UPPP done, it was covered via Aetna PPO without having done CPAP or even CPAP sleep study beforehand. I never had any issues with insurance at all (in stark contrast to my constant calls to my HMO nowadays).

Then again, maybe I just am lucky (in a weird, not really lucky way) with insurance - I just got a heated humidifier covered by HealthNet, which apparently doesn't cover heated humidifiers.
