Considering how serious your immediate condition is, you might opt for a trach. This doesn't have to be a permanent choice since a trach is reversable. Later on you might want to explore other options/surgeries which will open up the obstructions at the level of the tongue. None of the surgeries you did before (uppp, turb., septoplasty) did anything for the tongue-obstructions which usually CAUSES the apnea. The other stuff you did only treated the peripheral areas (nose and palate) which usually only AGGRAVATE (but doesn't cause) the apnea.
