Right now, the trend seems to be towards the TAP, Thornton Adjustable Positioner. I have heard it is the "state of the art" but, it is a little pricey. As Patrick said, though, they either work or they don't. A lot depends on who fits it, the user, and how long you can deal with the frustration if it doesn't work. Check with your doctor.
