In my opinion, a possibility is that one is sleeping in apreperred position, usually on their side. (There are some papers on RDI and patients sleeping on their side, but they do not metion pain.)

My opinion is based on my experience. The outside of my right foot was painful. One night when I went to bed I found that I crossed my feet and that part of my foot was against the left foot. I have also had problems with both shoulders, my left hip and with my left foot. (I do not remember that both the left and right feet had problems at the same time.) I had surgery on my right shoulder and the pain trasferred to the left shoulder within six weeks. If you would like some references on positional sleeping, please let me know.
