I am not sure how things are going now but when I read this I had a little different take on it. I agree it is wrong for him to do what he is doing.
However, you mentioned that you have had a good marriage and you seem to love him.
A trach is a lifesaving change for you but it is also a big change for him and he may need time or help dealing with the change. If you want to save your marriage and your health at the same time you might ask him to go to counseling with you.
My wife was not happy with the appearance of my trach when I got it. It was a hole in my neck and it bled and oozed and that took her time to adjust to it. She however, never sad a thing about her discomfort. That may be good or bad depending on how you look at it.
