Any dental appliance experience?

I just got a Herbst dental appliance as CPAP didn't work for me. Has anyone used this device or simmilar? How many mm does one need to advance the appliance until they are at an optimum level? I'm at 3mm and I don't really feel better in the morning. Any thoughts, suggestions?
