I don't know if there is a "usual" I'd go for the best advice from a doctor you trust. I would not advise doing this in stages, though. I would definitely not go through this piecemeal,if I could avoid it. I would only do it if I could suffer once and get it all over with! Which I have! If you aren't confident with your doctor, and some would say even if you are, get a second opinion! But we are not equipped to relate anything but our own experiences here!
