Concerned With Sleep Problems

Alrighty.. so I have a multitude of sleeping problems, none of which have been treated or diagnosed. I was prescribed trazodone once for insomnia, however it didn't seem to help other than make me feel loopy. I have problems going to sleep, and once asleep I dream intensely. Very vivid, life-like dreams, I wake feeling as if I've been awake all night in this dream and am usually cranky and very tired in the morning. Also, for the past few months I've been having night sweats. My significant other has commented on how sweaty and hot I am in the middle of the night. I do not have this problem during the day, and no matter what I do, I always have these sweats. It doesn't matter what I'm wearing, what the temperature is in the house, how the bedding is or anything. I'm wondering if I should get checked out. I always have dark circles around my eyes, I wake up almost every hour during the night. I can sleep for days on end if given the chance. I also get extreme headaches throughout the day and wonder if these are attributed to the sleeping problem. If anyone has any suggestions or ideas, please let me know. It would be nice to sleep.. I've always had these vivid dreams, as far back as I can remember. Help!!
