Sure sounds like an airway collapse to me, but a sleep study would tell for sure. Definitely do it, your health is important.

There are a few other things that can cause choking during sleep like laryngeal spasm but it's much more likely to be a simply airway failure due to tissue collapse.

To answer your question directly, yes, your brain will probably always wake you up when this happens, but the stress of the event and resulting bloodpressure spike could cause heart problems or stroke.

And, yes, you should be worried, so tell your Dr you want a sleep study and don't let them tell you you aren't overweight or whatever - insist.
