I really think a sleep study is called for based on what you described. You should find an acredited facility and a doctor that specializes in sleep disorders, don't rely on someone other than a specialist. If I'd have listened to my primary care doctor, he'd have me diagnosed with depression and that's it. Thank goodness for my Psychiatrist at the time, she had the foresight, albeit after several years, to send me to a sleep clinic.
Sleep deprivation can manifest itself in numerous ways and for me, I never seem to have anything that follows a text book so it always takes me a lot longer to get diagnosed and treated.
Much of what you described reminded me of myself before I found a successful treatment.
In addition to getting a good sleep specialist and sleep clinic, you should also educate yourself. There is a place on this website where they list reading materials and I suggest that you go there and pick a few to start with.
