What is wrong with me?

I can't even find a name for it. I get to sleep fine but I wake up continually during the night, say every 1 to 2 hours. I don't have trouble getting back to sleep, but I wake up as tired as went I went to bed.
I am on call for work of a night time and a couple of nights a week have to get up at 0230 hours for an hour or two for work. We have recently had a baby but I am sleeping in another room to try and get some sleep. I have had the same job for over 5 years, but it is only recently this problem has started, say a month ago. But I am exhausted.
Can someone please help me or at least tell me what this disorder is called?
