Mike,

As a sort of parallel to your issue, you might want to go to a different doctor in your area (or within a reasonable commute distance) for a second opinion. It sounds like your doctor doesn't really have any experience with dealing with your problem and isn't really bothering to "go the extra mile" to educate himself to help you further.

Also, have you tried any prescription sleep aids like Ambien?
