In Australia we pay 35% penalties in the auxiliary health care industry for the 23/7 shift. I have recently stunned my staff (I'm new) by refusing to accept that the day when the overnight shift ends can be counted a day off. It definitely is not as you work seven hours. Some staff have been working what I consider very poor rosters - only ever having one day off instead of two in a row. etc. They are not all happy when contemplating the changes, but so far those who have accepted them are saying that after a few weeks they would not like to go back to the old rosters.
I believe if rosters follow healthy guidelines, then staff have fewer days off sick, and are able to be more effective on shift.
