Troy, I sympathize with your bladder! A lot of folks are going to be up a creak. We had our 5 year re-visit last July, and our site doc used a modified version (this was not formal then). Our package was done using the old application, however, we have several RPSGT's, several boarded physicians, and a rigorout QA program (we are hospital-based) so we did fine. Look at your weaknesses and honestly see what you can fix. I have been through this four times, so let me know if I can help. Just e-mail me.
