pickwickian patient

How low of SPO2's do you tolerate with this type of patient? Almost 400 pounds. Resting SPO2 without O2 was 80%. Put O2 on 3lpm to get to 88% prior to study. During the study when the patient hits REM his efforts are so weak that I have to start a back up rate and then I've got pressures of 22 over 7 with O2 bled in at 4 lpm to get SPO2's around 81-83%. Tried him on his side but didn't seem to help.

Any ideas about how to further treat him?

Thanks
