My guess is that this patient has a degree of congestive heart failure, causing V/Q imbalances and shunting.
In other words, no matter what you do, this patient probably needs at least a diuretic to get rid of some fluid in his lungs. You, sir, have done about all you can do with PAP and supplemental oxygen.
