You did the right then by instituting CPAP. What conserns me is the statement that CPAP, O2, and BiPAP didn't do any good. How high did you go with the pressures and did you make sure he wasn't venting from his mouth or around the mask. You should be able to improve ventilation even with severe OSA patients, to the point of getting SaO2 levels close to normal. It sounds like events continued, so I would encourage you to look at mask interface problems and the use of a tight chin strap.
