I had a major argument with a former supervisor on this subject....

If 18, 19, 20 cm is "too high", why do manufacturers make CPAPs that go that high?

I have titrated patients to 22 cm CPAP only....highest BiPap I have done was 25/17.

Needless to say, humidity is a given at these pressures, but if they eliminate apneas and the patient can actually tolerate it, then thats what they need.

If a patient is intolerant at higher levels, but were ok at say 12-13 cm, then it becomes a matter of how reduced the RDI is. If a pt. has a baseline RDI of 80 and its reduced to 8 @ 14 cm with good tolerance, and RDI @ 0 at 19 cm, but patient can't stand it, then its better to have a few events and increased tolerance than to have them be completely intolerant and as a result non-compliant.
