There should not be limits set for CPAP pressures. Most patients will tolerate the level of pressure needed to support their airway. Titrations that go up to 18 to 20 or more should begin to show improvements in ventilation at some lower pressure with gradual elimination of the obstruction as the pressure reaches the theraputic level. If you don't notice this change as you are going up then it is likely that mask leak or mouth venting is the problem. I put one patient on 20 cm/H2O 13 years ago and he is loving it to this day. He knows that he would not be with us if he didn't use it.
