Usually, and I stress usually because all private insurance companies are different, another study can be done if patient has new or increased severity of symptoms. If prior to patients first study their Epworth was say, 11 and now its 14, you have a good case for doing a new study. If pt. had no complaints of EDS before and now states he/she can't stay awake, you have new clinicals for a new study.
Medicare should be the same way, but again, different regions have slightly different protocols. Check for any LMRPs in your region.
