I always liked to error on the side of doing the MSLT for cases of mild OSA. The data ruling out Nacolepsy, and establishing a sleepiness level which is measureable is very valuable. We established a protocal which called for doing the MSLT anytime the patient didn't meet criteria for starting CPAP in the first night. Remember that even though the patient might be diagnosed with OSA it is nice to have the results from the MSLT in case you need to evaluate treatment benefits or other criteria which would require MSLT testing post CPAP initiation. Now you are going to have the initial test results to make a comparison.
