i've tried all the methods you've mentioned, sometimes they work...sometimes they don't.
the bottom line is if you use cpap/bi-pap, your always gonna have a leak, either from headstrap shift from head movement or mouth leaks. all you can do is keep the leak level low and focus on arousals and o2 saturations. i'm a firm believer in giving patients an extra cm h2o prssure to compensate for leaks. compliance problems are not related to high pressures so much as a lack of patient preparation for the treatment and the distaste of patients to sleep with a device.
