An extended montage can be valuable in this circumstance to:

1. Prove any clinical behavior is NOT Sz related.
2. Enhance the all important EMG recording. With RBD separate channels for each arm and leg yeild much more information on actual EMG activity. At points in the recording of RBD the primary EMG activity may not be exhibited in the chin or leg leads. It may be in the arms, etc.

An adequate RBD montage is a double distance longitudinal bipolar EEG derivation with regular scoring channels plus a 2-wire separate channel for each limb. So you will be running 5 EMG channels. Its quite impressive to see an RBD person have clinical behavior with this montage.

It only takes an extra 15 minutes to hook up this montage if you use paste. With RBD people, I always recommend Collodion, if the patient does not mind.

Principles and Practice of Sleep Medicine, Second Edition, pages 577-583 shows 5 extended montages specifically for RBD, four of which have the 5 EMG channels and one has 10 EMG channels. It also explains why the additional EMG channels are necessary.
