There will be lots of ways to cut corners in medicine. I can diagnose OSA with a H&P with 90% accuracy and 80% sensitivity! In fact, take a fat over 40 male that snores and just divide the neck circumferance in half and give him a CPAP set to that pressure. You will help a lot of patients but it will not be quality medicine. Most patients with fever can be treated on the phone by just calling in a perscription for antibiotics and do pretty good. There are 89 sleep diagnoses that we deal with and we are professionals. The total cost for a lifetime sleep disease is still less than a colonoscopy or galbladder workup. You also have to factor the missed disease, cost of retesting and the psychological effect of not doing a full court press with someones health at stake. No good deed will go unpunished.
