Every MSLT needs to be done after an all night PSG. This is to document that the Pt. had an adequate night's sleep prior to the MSLT being done. A person who is sleep deprived in any way will have a false positive on the MSLT. If you went to a truck stop, or heck the medical Residents in say their 20th hour of duty could have sleep onset REM.
With our lab the PSG the night before has got to be within normal range. That means RDI 75 -80%.
If a Pt. is suspected of having both OSA & Narcolepsy, we will treat the OSA first with CPAP, then bring them back for follow-up and possible PSG/MSLT.
I think it can be a misused test if performed for other than the diagnosis of Narcolepsy.
