First we must set the parameters, such as what a portable device MUST measure and how to be called a portable PSG versus a respiratory screening device and whether setting up a self titrating CPAP is a diagnostic tool or not. Your comment about research unattended studies being of higher quality than one would think: the technologists in research labs are more involved in getting better outcomes than techs in diagnostic labs.

We need the proper tools for the situations to do appropriate testing - who is going to get us these tools or determine what those tools are?

There will always be a need for full PSG centers diagnosing narcolepsy will still require 20 hours in a sleep lab just to mention one of the eighty some diagnoses that can come out of a PSG.
