BC/BS is requiring an Epworth score of 12

Virginia BC/BS is requiring an Epworth score of 12 to get auth for PSG. This seems very high to me. Other carriers are looking for > 8. We have been able to get auths for the < 12 BC/BS pts but we have to appeal the first decision. One pt stated he fell asleep driving and had witnessed apnea by his wife. His ESS = 11. He was denied the first time just because of it not being 12!
