Normally in our lab if a pt. is here for CPAP titration and in the middle of the test they refuse to continue with the CPAP, we take the CPAP off and begin what we call "conservative management" by raising the head of the bed, give the pt. oxygen at 2 liters per minute and have pt. sleep in the lateral positions. While this may or may not have a positive effect on the pt.'s OSA at least you won't waste the pt.'s time or money by simply doing another baseline study when you already know they are positive for OSA and you would be able to rule out for the doctor and/or the pt. that these efforts work or not. Of course you would need to get permission of the medical director or the pt.'s MD first before doing these things, but in our lab this is part of our policy and procedure. Just thought I would throw this out for consideration.
