OSA , oops its snoring with C-Stokes!

Posted by ksleep on May 27, 2004 at 20:59:30:

Had a patient with snoring with regular apneas. Tech sets up CPAP. Snoring resolves and then its obvious that the apneas are Cheyne Stokes. The CPAP did nothing to relieve the apneas and desats. Maybe it decreased the arousals form the snoring.

Does the patient qualify for CPAP? Does the patient need CPAP? Does bilevel really help for centrals (I've been told it does but it doesn't make sense to me if they don't initiate a breath)? Should I just send them home on O2 and do a follow up nocturnal oximetry?

Any thoughts?
